RTM - In Plain Terms
What is it?
RTM is its own set of Medicare billing codes, separate from psychotherapy. Your practice bills monthly, per client, for between-visit support: setting them up on a tool, the client doing CBT-based check-ins or exercises, and a clinician reviewing their progress and following up.
It's billed on top of the sessions you already provide, so you earn more from your existing Medicare caseload without adding sessions. It does take some clinician time each month, and we'll be transparent about what that looks like and what the numbers were for a practice doing it.
Why now?
RTM has been around for a few years, but the old rules made it tough to use with therapy clients.
You needed at least 16 days of client engagement in a month and 20 minutes of clinician time before you could bill. Starting in 2026, Medicare added new codes for months when a client engages 2 to 15 days, plus a 10-minute management code for shorter check-ins.
That's a much better fit for how therapy clients show up in the middle of busy, messy lives.
